Your surgeon will examine your nose and hold a discussion about the purpose of the operation, as the approach and surgery undertaken will depend on whether the purpose is medical or cosmetic.

Moreover, there are different types of rhinoplasty that can be performed with changes to; the ridge, the tip, the width, the bridge, the length or the septum.

The meeting will last 20-45 minutes and the surgeon will be at your available to answer any questions you may have or allay any fears that might be causing anxiety. The surgeon will also discuss how the operation works, the aftercare involved and what results can be expected.

Pre-Operation

We will invite you to attend a pre-operative assessment. Your nurse will invite you to take part in a series of medical checks to ensure you are healthy and fit enough for the surgery.

All patients will be tested for MRSA and there may also be some blood and urine tests administered, however, there’s no need to worry, we just want to ensure you are getting the best possible service from Pall Mall.

The most important part of this pre-rhinoplasty assessment is taking photos of your nose so that we have the medical records of how it looked prior to the surgery.

This way, we will be able to successfully show you your transformation as well as keep track for medical purposes. The nurse is there to help you during the appointment so ask questions so that you can feel confident about your procedure.

Surgery

Upon your arrival at our state-of-the-art private hospital, you will be shown to your spacious and comfortable en-suite room. Your surgeon will then come to see you and make sure you’re in good spirits and health for the surgery before asking for your consent for a final time.

The team will then take you into the theatre for the operation, which will be around 1.5 to 3 hours. During this time you will be sedated with a general anaesthetic.

Post-Surgery

We offer a range of post-surgery appointments for our patients including a 1 week with the nurse to remove your cast and a 6 week with your surgeon to check that you are happy with the result. Our patient experience team is here to support our patients after their cosmetic surgery. They will call you post-surgery to see how you are recovering, get you booked in for your one week post-op appointment and ensure you have all the information you require to aid your recovery at home.

From a personal perspective, you should look to stay away from social areas for around 2 weeks after the surgery to avoid catching a cold, whilst strenuous exercise and heavy lifting needs to be avoided for 4-6 weeks.

Our team also recommends resting in an upright position for at least a few days after the op.

Aftercare

We like to get our patients back in after the surgery twice within the first 6 weeks, so don’t stress if you’re worried about post-surgery recovery. In addition, our nurses will check on your progress as you may experience bruising, swelling or numbness.

How to prepare for rhinoplasty surgery

All nose surgeries will involve initial consultations with a qualified surgeon. During these consultations, you should be prepared to answer questions about your surgery goals and your medical history. This will help inform the surgeon on the best approach to your rhinoplasty surgery - as mentioned, there are different types of rhinoplasty, so it is important that you answer truthfully and to the best of your knowledge.

During the consultation, your surgeon will discuss each step of the process, from the pre-op to the post-op of rhinoplasty. Don’t be afraid to ask them questions that you may have - our Pall Mall surgeons are always happy to provide advice!

If you're eligible for rhinoplasty, you will be invited to a pre-operative assessment where you will undergo medical checks to make sure that you are medically fit for the surgery you desire. Consent at every step

At every stage of the pre-op, our team will ask for your consent to ensure that you are still willing and aware of what it is you are agreeing to. We do this so that you are always empowered to withdraw from the surgery at any point before the procedure - this is completely within your right!

Before the surgery

It is recommended that you avoid smoking, drinking alcohol, putting topical creams on your face, and taking ibuprofen, aspirin or even vitamin E before your surgery. Your surgeon and nursing team will inform you on pre-op instructions prior to the actual surgery, so it is important that you listen to them.

Pack all of your cosmetic surgery essentials:

  • Spare clothes (loose and easy to take on and off) including pyjamas and underwear
  • Identification
  • Your phone and charger
  • Toiletries
  • Travel pillow

The risks to be aware of

As with any type of surgery, cosmetic or non-cosmetic, there are risks that every patient should be aware of before proceeding. When it comes to rhinoplasty and its variants, risks include:

  • Nosebleeds
  • Altered sense of smell
  • Numbness and/or stiffness
  • Blocked nose
  • Asymmetry
  • Bruising
  • Skin discolouration
  • Scarring
  • Swelling
  • Damage to nose cartilage or nasal septal perforation (hole in the nasal septum)
  • Breathing difficulties

There is also the small possibility of:

  • Infection
  • Allergic reaction to anaesthesia
  • Heavy bleeding
  • Blood clot

Although some of these, such as bruising and swelling, may eventually fade over time, more severe risks like allergic reactions and infection are more serious. However, all of these risks and potential complications will be discussed with you in depth before you finalise your decision in going ahead with the operation.

Note: Should you wish to proceed with any additional surgery following your rhinoplasty, you must wait until your nose is fully healed from the first operation.

On average, it may take up to 12 months for a patient to fully recover from a nose surgery.

What are the different types of rhinoplasty?

Nose Reshaping Surgery

Nose reshaping surgery is standard rhinoplasty. For those with a crooked nose, flat nose, or upturned nose, nose reshaping surgery can help with reshaping the nose to the desired shape that you want.

For nose reconstruction, your surgeon will simply break the nose bone and rearrange the bone and cartilage in the position that you desire. This may be done for those with a dorsal hump. As painful as this sounds, do not worry - you will be under general anaesthetic throughout the process.

Whether your desired look is suitable for rhinoplasty - according to your current facial structure, etc. - will be discussed with you during the initial consultation with your surgeon. You may find that your surgeon will recommend a similar but different look, and although the outcome is not what you had in mind originally, it would be best to trust your surgeon as they will have clocked years of experience in the best route to follow. Regardless, we will ensure that you’re fully satisfied with the look of your reshaped nose prior to the surgery.

Nose Reduction Surgery

When it comes to making your nose smaller or narrower, nose reduction surgery (also known as reduction rhinoplasty) can help with this. Nose reduction reduces the size of the nose by removing pieces of cartilage or bone from the nasal area.

However, when it comes to making the base of your nose appear naturally narrower, alarplasty surgery is an appropriate option.

Full recovery from alarplasty can take up to 1 to 3 months.

Nose reduction and alarplasty are not the same procedures, but their purpose in reducing the size of the nose is similar.

Nose Augmentation Surgery

The complete opposite of nose reduction, nose augmentation refers to rhinoplasty that enlarges the shape of the nose. Nose augmentation can widen or lengthen the nose, from the bridge of the nose to the tip and nostrils.

This is done by taking cartilage from your ears and/or bone from other parts of your body such as the hips, elbow, possibly even your skull, for the cartilage or bone graft. With these grafts, your surgeon will reconstruct the shape of your nose, building it up into the shape that was planned.

Nose augmentation rhinoplasty is a popular option for people who believe that the size of their nose appears too small for their face. Even a slight change to the size of the nose can make a big impact on a person’s face and to their self-esteem.

Tip Plasty Surgery

If you have a downturned nose, or it is specifically the tip of your nose that you are unhappy with, tip plasty surgery is a good way to alter the nasal tip.

In comparison to standard rhinoplasty, tip plasty is often much easier and the recovery time is relatively shorter. This is because there is no reconstruction of the nasal bone and cartilage on the bridge of the nose, which causes the swelling, bruising, and bleeding in standard rhinoplasty surgery. As a result, recovery from tip plasty is less demanding.

If you are wanting to change the look of the tip of your nose only, then tip plasty surgery is certainly recommended compared to a normal nose reshaping surgery.

During the tip plasty procedure, your surgeon will make an incision between the nostrils - an open rhinoplasty. The bulbous tip cartilages, nostrils, and skin of the tip can all be altered, refining the shape of the tip. This also means that this procedure is suitable for those with nostril asymmetry as the nostrils can also be evened out with tip plasty surgery.

Despite the differences between rhinoplasty and tip plasty, as they both usually require an open rhinoplasty, the probability of scarring and the type of scarring are also the same.

Septoplasty

Those with facial birth defects, particularly a cleft lip or cleft palate and a deviated septum, would be most appropriate for septoplasty. A deviated septum can also be caused due to an injury and can result in difficulty breathing as one nasal passage is smaller than the other.

With septoplasty surgery, this can be corrected for both cosmetic and medical purposes. Septoplasty can improve breathing, relieve facial pain, and reduce the frequency or likelihood of nosebleeds - all of these symptoms are due to a deviated septum.

The procedure for septoplasty is fairly simple, but like with any rhinoplasty, a complex procedure in its own right. Typically, your surgeon will make an incision on one side of your nose, lifting up the mucous membrane to access the septum. The septum is then moved into the correct position, and the mucous membrane is also repositioned.

Your surgeon may also remove pieces of bone or cartilage that are in the way for better access and reconstruction. Stitches may be required so that your septum and mucous membrane are secure during the healing process. Alternatively, stuffing cotton balls into your nose may also be suggested to keep them in position.

Alongside a septoplasty, you may also be recommended by your surgeon to undergo a nasal valve repair should they find any nasal obstruction during their consultations with you. Nasal valve repair surgery fixes nasal valve collapse, using a small piece of cartilage (usually from your ear) to support the area inside your nose that is collapsing to help with airflow.

This will result in a minor change in the appearance of your nose. As the cartilage graft is not particularly large and the change more subtle, this surgery may be right for you if you’re looking to not have your nose altered drastically.

What does the rhinoplasty procedure look like?

The procedure varies depending on the type of rhinoplasty that you opt for. However, with a standard rhinoplasty, the procedure is quite simple.

Anaesthesia is applied

Firstly, general anaesthesia will usually be administered, which will make you unconscious throughout the surgery.

Incisions are made to prepare the nasal area

Whilst under anaesthesia, your surgeon will make incisions between (open rhinoplasty) or inside (closed rhinoplasty) your nostrils, separating the skin from cartilage or bone to begin the reshaping process.

Although a closed rhinoplasty involves less swelling and no scars, it is not always an option that is possible.

Nose surgery is performed
  • For a nose reduction, some cartilage and bone will be removed to make the nasal area smaller once healed.
  • For a larger nose (nose augmentation), some cartilage and bone may be taken from your ear to build up the nasal area (known as a ‘graft’).
  • For nose reshaping, the nose bone will be broken whilst you’re under general aesthetic to enable the cartilage and connective tissue to be rearranged to your desired look.

Your surgeon may put a plastic or metal splint on your nose (or a splint or nasal pack inside your nostrils), stabilising your septum to help it conform to its new shape as it heals.

The procedure time is estimated at an initial 60 minutes - though times could vary and this will all be fully discussed with you during your initial consultation.

Recovery & Aftercare

Rhinoplasty is a day case, meaning you do not need to stay overnight at the hospital but it is advised that someone else drives you home as you may still be affected by the anaesthesia.

What is the aftercare process like?

After your nose surgery, regardless of the type of rhinoplasty that you had, the recovery and aftercare process is extremely important. You may find that you may experience swelling, numbness, and bruising, but these are all common during post-surgery due to increased blood flow to your nose after surgery as your body works to heal the visible scars.

After a week, your splint should be able to be removed by a specialist. It is recommended that you avoid going outside for at least 2 weeks after the procedure and completely stop or avoid activities for 4 to 6 weeks such as:

  • Running and other strenuous physical activities, e.g. swimming
  • Blowing your nose until the splint is removed
  • Sneezing - whenever possible, try sneezing through your mouth
  • Chewing excessively
  • Excessive facial movements, e.g. laughing and smiling
  • Excessive sun exposure - prevents skin discolouration around the nose
  • Wearing glasses
  • Pulling clothes over your head
  • Hot baths

Rhinoplasty recovery

You may find comfort in elevating your head above your chest using multiple pillows or resting in an upright position to help with reducing swelling and bleeding for the first few days after the operation.

Whilst you could possibly recover from rhinoplasty surgery in just over 2 weeks, this varies from patient to patient. It may take several months before you notice the intended impact of the surgery, and potentially six months for swelling to completely disappear.
